A production directory would be maintained with the hospital and community partners. This prototype uses illustrative entries to demonstrate how a Navigator could search resources without browsing the open web during every patient interaction.
Closed-loop habit
Do not stop at “resource provided.”
The Navigator workflow should record whether the patient could reach the resource, whether it fit the need, and whether the next step was completed or handed off.

Help the patient reach the resource using the preferred communication method.
Verify
Confirm whether the service is available and what the patient must do next.

Check whether the practical service was actually arranged or accessed.
